This study aims to comprehensively analyze the influence of Digital Leadership, ICT Competence, and Organizational Support on the Organizational Performance of Kardinah Regional General Hospital, Tegal City through the Effectiveness of Digital Application Use as an intervening variable. This explanatory research study uses a quantitative approach. The study population includes all 950 employees of Kardinah Regional General Hospital (724 medical/paramedical personnel and 226 non-medical personnel). The sample was calculated using the Taro Yamane formula with a precision of 5%, resulting in 300 respondents. Data were collected through a structured questionnaire with an interval scale of 1–7 and analyzed using Structural Equation Modeling (SEM) based on AMOS 23 and the Sobel Test to test the mediation effect. The results of the structural analysis show that Digital Leadership (β = 0.630, CR = 5.908, p < 0.001), ICT Competence (β = 0.667, CR = 6.767, p < 0.001), and Organizational Support (β = 0.741, CR = 6.669, p < 0.001) have a positive and significant effect on the Effectiveness of Using Digital Applications. The Effectiveness of Using Digital Applications has a positive and significant effect on Organizational Performance (β = 0.800, CR = 5.587, p < 0.001). On the other hand, the direct effect of Digital Leadership (β = -0.011, p = 0.915), ICT Competence (β = -0.018, p = 0.855), and Organizational Support (β = 0.094, p = 0.383) on Organizational Performance is proven to be insignificant. The Sobel test confirms that the Effectiveness of Digital Application Use acts as a full mediating variable on the influence of Digital Leadership (t = 4.000), ICT Competence (t = 4.108), and Organizational Support (t = 3.439) on Organizational Performance. Practical implications emphasize that optimizing digital applications (SIMRS & RME) is the main key to achieving superior hospital performance.
